Shorter form?

I hear the pts. grumbling about "more paperwork", and the thought occurs...Make the form shorter-maybe just a 1-10 rating, chief complaint(or reason for them being in the clinic), and a box for comments. Since there appears to be no State requirement, make it a breeze for the pts. while acknowledging the state of the patient's wellness. Many, many say they are in the clinic for wellness, not for pain.

- There is a state requirement
- There is a minimum standard of care for the profession
- we have to make sure we are doing at least the minimum...
- We don't create the forms, corporate does....and they require them.
